Request for Research Participants
Researchers at the University of Illinois at Chicago are looking for children with disabilities aged 12-18 and their parents/caregivers to complete an internet-based survey on the lifestyle and environment of adolescents with disabilities. This survey, comprised of sections to be completed first by the parent, then the teen, can be found by visiting the following link: http://www.healthforyouth.org and entering the access code: CVS1. Please feel free to preview the survey; we only ask that you to enter: evaluator as the access code so that we can separate your responses from those of the actual survey responses. Please contact Brienne Davis in the Department of Disability and Human Development at bdavis7@uic.edu or 312-355-4054 if you have any difficulty accessing or completing the survey.

We are fielding this survey nationally and would greatly appreciate your help in ensuring that families with children with various disabilities are properly represented. If you should agree to lend your assistance, we are hoping you would be kind enough to share this information with other families or relevant groups and organizations.
